Abstract:
Floating, Production, Storage and Offloading (FPSO) is a type of installation that
becomes very important nowadays. This facilities provides process and store the
produced hydrocarbons. Everyone on board of the vessel must always put priority on
safety to avoid any kind of hazards to occur. One of the hazards is fire and explosion.
Lessons learned should be taken from previous accident in order to prevent the same
incident to happen in future. Some past accidents that could be learned from is the
hazard occur on FPSO Maersk Ngujima-Yin and FPSO Cidade de Sao Meteus. From
that accident, factors that cause and lead to fire and explosion could be also identified.
For future prevention, it is crucial to precisely follow the rules and regulation and also
regulary check all the fire fighting appliances and system to ensure that they are in
very well maintained condition. At the end of this research, there is also provided
some of the idea or suggestion that might be used in future design and operation of
FPSO for a better performance thus reduce the number of personnel injuries or
fatalities.
